Through trusted relationships and community-based support, we partner with health plans and local organizations to help members navigate healthcare, stay engaged in care, and live healthier lives.</p><p>We are launching a new clinical pilot that integrates physicians, care navigators, and community support teams to create a more coordinated and human approach to care.</p><h3>About the Role</h3><p>We are seeking a Pennsylvania-licensed physician to serve as a consulting clinician for an early-stage care program. In this role, you will conduct initial virtual visits with members to understand their medical needs, barriers to care, and overall health goals.</p><p>Your evaluation helps set the direction for the member’s care journey. After the initial visit, ongoing support is provided by a care team including Community Health Workers and Licensed Clinical Social Workers, allowing you to focus on clinical guidance rather than longitudinal case management.</p><p>This role is ideal for physicians interested in innovative care delivery, telehealth, and population health models that extend beyond traditional office-based practice.</p><h3>What You’ll Do</h3><ul><li>Conduct initial telehealth visits with enrolled members</li><li>Assess medical needs, care gaps, and barriers to accessing care</li><li>Establish a clear clinical plan and care priorities</li><li>Collaborate with care team members to ensure a smooth handoff and continuity of care</li><li>Provide clinical insight to help refine program workflows during the pilot phase</li><li>Support a patient-centered approach addressing social and medical needs together</li></ul><h3>Requirements<p></p><p><strong>What You Bring</strong></p></h3><ul><li><strong>MD or DO with an active, unrestricted New Jersey license</strong></li><li><strong>Board certified or board eligible (Family Medicine, Internal Medicine, or related primary care specialty preferred)</strong></li><li><strong>Comfort working with Medicare, Medicaid, or complex populations</strong></li><li><strong>Strong communication skills and ability to build trust quickly in a virtual setting</strong></li><li><strong>Interest in care coordination, health navigation, or whole-person care models</strong></li><li><strong>Comfort working in a startup-like, evolving program environment</strong></li><li><strong>Telehealth experience preferred</strong></li></ul><h3><strong>Benefits</strong></h3><ul><li><strong>1099 independent contractor</strong></li><li><strong>Part-time, flexible scheduling</strong></li><li><strong>Virtual visits</strong></li><li><strong>Pilot program with opportunity to expand as the model grows</strong></li><li><st
